The Florida Rural Health Access Network (FRHAN) is a health transformation and grant management organization focused on expanding access to care, strengthening rural provider networks, improving health outcomes, and supporting sustainable value-based health initiatives throughout underserved communities in Florida.
FRHAN’s work is organized around three federally aligned initiative tracks, each designed to advance the strategic goals of the Rural Health Transformation Program and deliver measurable, sustainable improvements in care across Florida’s rural and underserved communities.
FRHAN helps rural providers move from volume to value through accountable, outcomes-driven care models. We support the design and implementation of value-based arrangements and alternative payment models that reward quality, lower the total cost of care, and shift services to the most appropriate, lowest-cost settings – strengthening both patient outcomes and provider financial sustainability.

The Rural Health Transformation Program is a historic $50 billion federal investment, authorized under Public Law 119-21, that empowers states to strengthen rural health care access, quality, and outcomes over a five-year period. Funding flows through each state’s approved rural health plan.
FRHAN serves as a coordination and management partner for Rural Health Transformation initiatives in Florida – helping rural hospitals, provider groups, and community organizations design, launch, and sustain programs that meet the Program’s strategic goals while reducing the administrative burden on participating providers. Our three initiative tracks – value-based care, satellite clinics, and acute and specialty services – are built to align directly with these federal priorities.
